One Year Before Procedural Compliance Self-Assessment Year

This year is one year before your district's required year for the Special Education

State Performance Plan (SPP) Indicator
#14 assesses the outcomes of students with disabilities one
year after exiting their secondary placement. You will use the post
high website to complete all of the requirements of Indicator #14. All
data collected will be via the Wisconsin Post High School Outcomes
website. All Survey Instruments and
Survey Directions are provided
on this website.
The students who will be included in Indicator #14 data are currently
in their last year of high school, and will receive a telephone
interview approximately one year after exiting high school.
Required Activities for the Year Before Procedural Compliance Self-Assessment
- Copy the attached Student
Contact Form and complete a hard copy of that form for each
student with a disability who is currently in their senior
or last year of school and who you anticipate will exit through
graduation with a diploma, certificate of attendance, reaching
maximum age of attendance or by dropping out.
Keep this exiting Student Contact information until it can be entered into the Post High School Outcomes website in February of next school year. This will be your district’s Procedural Compliance Self-Assessment year. WDPI will collect your district's Indicator #14 data directly from the Post High School Outcomes website.
