Procedural Compliance Self-Assessment Year

State Performance Plan (SPP) Indicator #14 assesses the outcomes of students with disabilities one year after exiting their secondary placement. You will use the post high website to complete all of the requirements of Indicator #14. All data collected will be via the Wisconsin Post High School Outcomes website. All Survey Instruments and Survey Directions are provided on this website.

The students who will be included in Indicator #14 data are currently in their last year of high school, and will receive a telephone interview approximately one year after exiting high school.

There is only one required activity for Indicator #14 for the 2006-07 school year.

Required for the Current School Year

  1. Copy the attached Student Contact Form and complete a hard copy of that form for each student with a disability who is currently in their senior or last year of school and who you anticipate will exit through graduation with a diploma, certificate of attendance, reaching maximum age of attendance or by dropping out.
     
    Keep the contact information until it can be entered onto the post high school outcomes website in February 2008. Students who exit your school this year will be contacted for a post high school interview in April - June 2008, which is the year following your compliance monitoring cycle year. WDPI will collect your district's Indicator #14 data directly from the Post High School Outcomes website.
