Completing Indicator 14:
Procedural Compliance Self-Assessment Year

You will use the information provided here if:

State Performance Plan (SPP) Indicator #14 assesses the outcomes of students with disabilities one year after exiting their secondary placement. You will use the post high website to complete all of the requirements of Indicator #14. All data collected will be via the Wisconsin Post High School Outcomes website. All Survey Instruments and Survey Directions are provided on this website.

Students who exited your district during the previous school year through graduation, earning a certificate of attendance, reaching maximum age of eligibility or by dropping-out will be contacted by St. Norbert College Survey Center for a telephone interview approximately one year after the student exited your school district. Former student responses will be used to report your district's outcomes results and fulfill the data collection requirements of Indicator #14.

Required Activities for the Year of Procedural Compliance Self-Assessment
The following are the timelines and activities that your district will follow this school year.

Timelines   Activities to be Completed This School Year
Fall   LEAs verify their ISES Federal Student Data Report (3rd Friday in September)
October-January   Collect or update the Student Contact Form or Student Contact Sheet (paper or electronic copy)
January   WDPI populates the statewide outcomes website database
February   Call former student to verify/find viable phone numbers
February   Complete the Usage Agreement and receive a user name and password for website access. Enter the student's verified phone number(s) and special comments for calling directly into the website
March-April   Personalize Student Letter and mail to former student along with WDPI Letter, inform WPHSOS when letters are sent and email mkampa@cesa10.k12.wi.us with the send date
April-September   St. Norbert College Survey Center contacts exiters for a telephone interview
After October 1st   Review district data and web reports following completion of interviews
October   LEA and statewide Indicator 14 data are gathered directly from the website
October   Statewide outcomes data area available on the website
