Prepare Contact Information

Enter / Edit Contact Information
The Federal Data Report includes student demographic information, but does not include student contact information. The former student telephone number is needed so St. Norbert College Survey Center knows where they are most likely to be able to contact the former student for the telephone interview. The former student address is needed so your district can personalize and mail the former student letters announcing the survey prior to the telephone interview.

For Compliance Year Districts
Directions for collecting student contact information to use in doing your survey one year from now:

1. Use the electronic version of the Student Contact Form (Excel). Save a copy to your computer and complete the requested information for each student with a disability who is currently in their senior or last year of school or who exited last school year and will be interviewed this school year. Keep this contact information until it can be entered into the activated portion of Post High School Outcomes website in March after the student exits high school. Use this Excel list to create mailing labels for letters going to students.

OR

2. Copy/print the Student Contact Form (word) and complete a paper copy of for each student with a disability who is currently in their senior or last year of school or who drops out during the school year.

For Districts Completing Indicator 14 or Districts Doing a Survey Voluntarily
Directions for doing the survey this spring:

  1. Log In to the secure section of the website
  2. Select Prepare Contact Information
  3. Select Enter/Edit Contact Information
  4. Select the correct Survey Year (the year the former student exited; default setting is the current survey year)
  5. Click edit next to the student's name
  6. Enter all contact information including special comments for calling the student
  7. Click Save
  8. Repeat 5-7 until all student information is entered for all those to be interviewed
  9. Click Download Student List (optional)
    • Save to your computer to update and edit the list easily
    • Print the Student list (excel) and review it for accuracy or missing information
  10. Repeat 5-7 with additions/corrections prior to the start of the phone interviews

Personalize / Mail Letters
Sending your former students a personalized letter announcing this upcoming telephone interview is critical to achieving a good response rate. Personalize the Student Letter template with name of someone the former student may be familiar with, such as a former teacher or principal.

The DPI Letter is written by the State Director of Special Education and explains to the former student why this information can be collected without their permission. These are both mailed to the former student 2 - 3 weeks prior to the telephone interview. You may wish to do a press release describing the survey.

Placing the Press Release in a local newspaper or district newsletter is optional, but increases response rate and awareness of the upcoming survey.

  1. Log In to the secure section of the website
  2. Select Prepare Contact Information
  3. Select Personalize/Mail Letters
  4. Select Student Letter
  5. Save a copy to your computer
  6. Personalize the letter on District Letterhead with the name of a former teacher or principal
  7. Print one letter for each former student to be interviewed
  8. Select DPI Letter
  9. Print one DPI Letter for each former student to be interviewed
  10. Mail the Student Letter along with the DPI Letter to each former student to be interviewed
  11. Select Press Release (Optional)
  12. Save a copy to your computer
  13. Personalize the Press Release with your district information
  14. Print and send a copy on letterhead to each desired area newspaper and/or school newsletter
