Indicator #14 - Post High School Outcomes Survey

Indicator #14: "The percentage of youth who had IEPs, are no longer in secondary school and who have been competitvely employed, enrolled in some type of postsecondary school or both within one year of leaving high school."

Data collection for Indicator #14 is a two-year process.

Year 1: One Year Before Special Education Procedural Compliance Self-Assessment Year

The Wisconsin Post High School Outcomes website (www.posthighsurvey.org) is used to complete all of the data collection and reporting requirements of Indicator #14. All data collected will be via the website and all Survey Instruments and Survey Directions are provided on this website.

The students who will be included in the district's Indicator #14 data are currently in their last year of high school, and will receive a telephone interview approximately one year after exiting high school to assess their participation in employment, postsecondary education and community activities.

There is only one required activity for Indicator #14 for this school year.

Required one year before the district's Self-Assessment Year:

1.Go to www.posthighsurvey.org, download and complete a hard copy of the Student Contact Form for each student with a disability who is currently in their senior or last year of school and who you anticipate or know will exit through graduation with a diploma, certificate of attendance, reaching maximum age of attendance or by dropping out. OR , if you already have contact information in a district location (e.g. electronic database), verify and maintain this information.
 
Keep this student contact information until it can be entered onto the Post High School Outcomes website, which is year of self-assessment.

Completing Indicator #14:
Procedural Compliance Self-Assessment Year

Students who exited your district during the previous school year through graduation, earning a certificate of attendance, reaching maximum age of eligibility or by dropping-out will be contacted by St. Norbert College Survey Center for a telephone interview. Former student responses will be used to report district outcomes results and fulfill the data collection requirements of Indicator #14.

Required During Self-Assessment Year
To complete the data collection and reporting requirements of Indicator 14, districts are required to use the tools and directions provided on www.posthighsurvey.org to record accurate former student contact information and send the former students a personalized letter describing the upcoming interview.

The following are the timelines and activities required this year:

Fall   LEAs verify their ISES Federal Student Data Report (3rd Friday in September)
October-January   Collect or update the Student Contact Form or Student Contact Sheet (paper or electronic copy)
January   WDPI populates the statewide outcomes website database
February   Call former student to verify/find viable phone numbers
February   Complete the Usage Agreement and receive a user name and password for website access. Enter the student's verified phone number(s) and special comments for calling directly into the website
March-April   Personalize Student Letter and mail to former student along with WDPI Letter, inform WPHSOS when letters are sent and email lyneseg@cesa11.k12.wi.us with send date
April-September   St. Norbert College Survey Center contacts exiters for a telephone interview
After October 1st   Review district data and web reports following completion of interviews
October   LEA and statewide Indicator 14 data are gathered directly from the website
October   Statewide outcomes data are available on the website
